e. Administrative Organization (LIC 309) 2624 This form must be completed when a corporation, limited liability company, or other association is applying for a license. The entity must identify the organizational structure of the corporation and all directors and individuals with 10% or more ownership in the corporation. Individual applicants need not complete this form. This form must be updated and submitted to CCLD each time there is a change in the partners, officers, or structure of the corporation. 2625 f. Monthly Operating Statement (LIC 401) 2626 This form establishes the estimated gross profit based on income from caring for children, less estimated or actual reasonable operating costs. Operating costs include expenses for food and supplies, staff salaries and benefits, insurance costs, and costs related the physical facility, such as property taxes, rent amounts, maintenance, and utilities. g. Balance Sheet (LIC 403) 2627 This form is a compilation of assets and liabilities to establish the equity of the preschool applicant(s). CCLD may conduct a credit check to verify the preschool’s financial ability to operate the facility. h. Financial Information Release And Verification (LIC 404) 2628 This form provides financial information, including checking and savings account numbers, that is verified by a bank or other financial institution. The preschool must submit separate forms for each bank or financial institution with which it does business. CCLD uses this verified information to ensure the preschool has sufficient resources to cover its operating budget for at least three months. i. Personnel Report (LIC 500) 2629 This form identifies all employees and planned employee positions, including each anticipated employee’s or position’s working days and hours. Listed positions should include director/administrator, teachers, and other staff already hired at the time of application. Positions for which a staff member has not yet been hired should be listed as “To Be Hired.” This form permits the preschool to list employees as either subject to a criminal background check or exempt from fingerprinting requirements. 2630 It should be noted that any staff person, volunteer, or employee who has contact with children, except for volunteers who are also relatives of an attending child, must be fingerprinted and approved. 2631 j. Personnel Record (LIC 501) 2632 This form is to be completed individually for each employee at the preschool. It identifies the employee’s background information, including previous employment, education, references, and qualifications. Preschools, however, need only submit the form to CCLD for the proposed site director, unless that individual is also the applicant. Other preschool employees should complete this form, but those forms need only be kept on file at the preschool.
