Chapter 21 – Child Care Centers/Preschools

Preschools should submit original signatures of all documents to the CCLD as it will not accept photocopies of signatures. Be sure to make copies of all submitted applications and supportive documents before sending them to the CCLD to ensure the preschool has a complete record of its submissions.

1. L ICENSING F ORMS Applicants must submit the following forms to the CCLD in order to obtain a Child Care Center license: a. Application For Child Care Center License (LIC 200A) 2619 This form identifies the applicant(s) and the facility for licensure, as well as other basic information including the director of the facility, the total number of attending children, their age range, and the days and hours of operation. This form also certifies that the applicant(s) understand the child care center statutes and regulations and will ensure the preschool’s employees are properly fingerprinted, checked, and certified. A new LIC 200A form must be submitted if, after licensure, the preschool proposes a change in its capacity determination or location. 2620 b. Applicant Information (LIC 215) 2621 This form identifies the education and business backgrounds of each applicant and must be completed separately for each individual applicant. CCLD will use this form to verify an applicant’s qualifications if that individual also intends to be the administrator or director of the preschool. CCLD will also check if any disciplinary actions were or are being taken against the applicant if the applicant previously held a child care center license, owned 10% or more of a licensed facility, or was an administrator or director of a licensed facility. c. Criminal Record Statement (LIC 508) 2622 All applicants and site directors must complete this form which requires disclosure of any criminal convictions. d. Designation Of Facility Responsibility (LIC 308) 2623 This form identifies the person authorized by the licensee to accept responsibility for the facility in the licensee’s absence. This individual must be present at the facility during operational hours if the licensee is not on campus. The designee is also authorized to receive CCLD or other legal documents on behalf of the facility. If the preschool is a corporation, the corporation’s board must adopt a resolution authorizing this delegation. An individual cannot designate himself or herself as a designee. If a preschool changes the designee, it must submit a new form to CCLD within 10 days of that change.
