The School has a duty to reasonably accommodate an employee with a disability so that he or she can perform essential job functions. The following facts indicate that you may have a disability: [describe facts, e.g., inability to perform one or more essential job functions; doctor’s note that states work restrictions; employee request for accommodation; indications that employee’s condition presents a current safety risk to self or others]. In order to determine if you have a disability, the School needs the following information from a doctor: 1) Whether you have a disability as defined under the Americans With Disabilities Act or the California Fair Employment and Housing Act; 2) whether you can perform the essential functions of your job, with or without reasonable accommodation; and 3) any suggested potential accommodations. As a result, the School has made an appointment for you as follows: [identify doctor name, address, and appointment time]. You will be paid for the time needed to travel and to see the doctor. You are directed to timely appear for this appointment. Attached to this letter is a copy of the letter that the School has sent to [name of doctor]. As you will see, the School seeks non-confidential information from the doctor regarding whether you have a disability or not, whether you can perform your essential job functions as described on the attached job description, and whether there are any potential accommodations that would enable you to perform your essential job functions.
