S AMPLE S TATEMENT IN THE E VENT E MPLOYEE R EFUSES TO R ESPOND TO Q UESTIONS OR S UBMIT TO I NVESTIGATIVE I NTERVIEW You should be advised that if you refuse to respond to questions or submit to this investigative interview you shall be considered insubordinate and subject to disciplinary action, up to and including dismissal because of the act of insubordination. All employees are obligated to truthfully respond to questions in workplace investigations. The question (repeat exact wording of question, or if that is not possible, the gist of the question) is directly related to this investigation (if necessary, state exactly how question is related). Your refusal to answer that question may result in punitive action for insubordination and misconduct should you persist in your refusal. Your refusal to answer questions is a direct violation of the following sections of the Personnel Rules:

(List all relevant sections, i.e., duty to cooperate in investigations, duty to obey lawful orders of supervisors, etc.)
