S AMPLE O UTLINE OF W RITTEN R EPORT
There are any number of ways to prepare a written report. We recommend the following general outline/organization for the written report:

•
Scope of Investigation
•
Methodology
o Witnesses Interviewed o Documents Reviewed o Factors Used to Assess Credibility o Standard of Proof
•
Summary of Findings
• Summary of Relevant Facts. The organization of this section depends on the complexity the facts
•
Findings and Analysis
• Conclusions re Policy/Rule Violations (if part of assignment)
•
Attachments
As to the findings, there should be a separate finding for each allegation. Each finding should represent a reasonable and fair conclusion based on the information collected, reviewed and analyzed during the investigation.
