FIVE: Consider Providing Training and Assistance  To maximize employee’s ability to improve  To maximize school’s ability to successfully discipline or terminate employee  To maximize school’s ability to successfully defend litigation SIX: Prepare Annual Evaluation  Final chapter in year-long review process  Chronicles employee performance during the entire evaluation period  Assess  Review supervisor & personnel files  Review all other relevant data  Review previous evaluations & goals  Address past and future goals  Annual evaluation forms should include:  Clear explanation of ratings  Comment section for each rating area  Ample space to include examples  Space to discuss employee’s major accomplishments  Space to discuss obstacles and challenges over the review period  Annual evaluation should:  State whether employee met goals  Compare job performance against job duties and performance standards

 Commend outstanding performance  Identify all performance deficiencies  Identify the significance of deficiencies on the school and its programs, students and/or community
