Preschools must also maintain a minimum amount of personnel records for all volunteers. These records must include the following: (1) The required health screening and report. (2) The required tuberculosis risk assessment or test documents. (3) The required immunization or exemption from immunization documentation. (4) If the volunteer is to be fingerprinted, a signed statement regarding their criminal history or documentation of either a criminal record clearance or a criminal record exemption. 27
