Check here if the Student does not have any medical or allergy issues that might impact his/her ability to participate in the Field Trip safely, and does not require any medication while participating in the Field Trip, including while traveling to/from the Field Trip.
Please note any important medical or allergy information that might impact the Student’s ability to participate safely in the Field Trip, including any limitation on medical treatment rendered to Student:

A special note to Parents/Guardians: (1) All medications that the Student may need to take while participating in the Field Trip, including traveling to/from the Field Trip, must be registered on this form; (2) All medications, whether prescription or over‐the‐counter, excepting those which must be kept on the Student’s person for emergency use, must be kept and distributed by the staff; (3) Parents/Guardians must provide more than the required dosages of any medicine because there may be limited access to particular types of medicine on the Field Trip. A. Authorization for the School to Administer Over‐the‐Counter (“OTC”) Medications Parents/Guardians may provide the School with OTC medications to be administered to the Student while participating in the Field Trip, including traveling to/from the Field Trip. Parents/Guardians must provide the OTC medication in the original container and labeled with the student’s name, medication name, amount (dosage), and time schedules.
