under the chaperones’ direct supervision 24 hours a day, and will be staying in rooms where the chaperones are not present. [INSERT IF RELEVANT: The School has informed us of the recommended vaccinations for travel to [DESTINATION] by the Centers for Disease Control, which are attached to this Agreement. I acknowledge that it is my responsibility to verify the appropriate vaccinations needed as stated in Section 10 of this Agreement, obtain vaccinations for Student, and that I assume any and all risks associated with the decision not to vaccinate Student.] 4. Acknowledgement of Itinerary. A copy of the itinerary for the Field Trip is attached to this Agreement. I acknowledge that the School has provided the educational objectives, program overview, transportation and lodging arrangements, as well as the detailed itinerary and requirements for the Field Trip to the Student and the individuals that have signed below. 5. No Exceptions to Group Travel Plans. I agree that the Student will travel with the School group according to the travel plans made by the School. I will not expect any exceptions to be made for the Student because of personal or family plans. 6. Student Conduct. I understand that the Student is expected to abide by all School regulations, including those in the student handbook, during the course of the Field Trip. [FOR OLDER STUDENTS: I acknowledge that independence, responsibility, and autonomy are a critical part of the Student’s learning experience. During the Field Trip the Student will be subject to limited supervision and will be held responsible for his/her own actions.] I agree that the School has the right, in its sole and absolute discretion, to terminate the Student’s participation in the Field Trip at any time for conduct deemed detrimental to the School or to the Student. 7. Early Return of Student. I understand and acknowledge that, if the School terminates the Student’s participation in the Field Trip for any reason, the Student will immediately be sent home with a chaperone at the parent(s)’ or guardian(s)’ expense, or parent(s) or guardian(s) may be required to travel to [DESTINATION] to pick up the Student at the parent(s)’ or guardian(s)’ expense. 8. Voluntary Release of All Claims. To the fullest extent provided by law, I voluntarily release, discharge, waive and relinquish (“Release”) all claims, demands and liabilities (“Claims”) against the School, its officers, trustees, directors, employees, volunteers, insurers, agents and representatives (collectively “the Released Parties”), including those arising out of ordinary negligence, that are in any way related to or arising from the Field Trip, except for those Claims arising out of the Released Parties’ gross negligence or willful misconduct. This Release also pertains to any instruction or supervision related to the Field Trip on the part of the Released Parties. 9. Release from Third-Party Liability. I understand that the School is not an agent of, and has no responsibility for, any third party, including without limitation any sponsor or entity, that may provide or schedule any services, hospitality, public or private transportation, equipment, training or activities associated with the Field Trip.
