[If applicable: The Employee Handbook section ____ requires that employee participate and cooperate in investigations and section ___ prohibits insubordinate conduct. Your refusal to answer questions is a direct violation of Employee Handbook sections ____ and my direct order that you answer the questions asked in this interview. Please be advised that if you refuse to respond to that question or other questions or refuse to continue with this interview, that violates your obligation to participate and cooperate in investigations and is an act of insubordination and will be subject to disciplinary action, up to and including dismissal.
