Prepare a written summary of the allegations and have the complainant sign it; or Ask the complainant to place his or her complaint in writing. If the complainant refuses to sign a written statement of the allegations or submit a written complaint, make a written record of the complaint with a notation of the complainant’s reluctance to place the complaint in writing. Write down an initial outline of the complaint. Review the allegations to determine what type of wrongdoing is being alleged and to understand the nature of the complaint. If the complaint is by an employee for harassment or discrimination, determine the protected status on which the harassment or discrimination allegations are based such as: Race; Color; National origin or ancestry; Sex (including gender and pregnancy); Age (40 years and older); Physical or mental disability; Medical condition; Religion or religious creed; Marital status; Military and veteran status; Sexual orientation; Gender, gender identity, and gender expression; and/or Genetic information. If the complaint is by an employee for retaliation, determine the protected activity on which the retaliation allegations are based such as: Opposition to discrimination or harassment; Filing of a discrimination or harassment complaint; Participating in a discrimination or harassment investigation; Making a request for an accommodation for a disability; Making a request for an accommodation for religious beliefs; Filing of a workers’ compensation claim; Filing of a wage and hour claim; and/or
