M ANDATED R EPORTING C HECKLIST All employees must receive and sign acknowledgment form informing them of their role as mandated reporter and copy of Penal Code sections Train employees (legal requirement for licensed child care facilities) Develop policy regarding reporting and whom employees can speak to about concerns Designate point person for reporting forms and CPS contact information If report is required: agree who will file report and ensure report is filed immediately (if your county has an online system for non-urgent calls, discuss whether you may make the required report through that system.) follow up in writing within 36 hours keep a copy of the written report in locked, secure location separate from pupil records or personnel files do not disclose report to any unauthorized recipients
