This is to invite you [for unionized employees with the right of representation: AND a representative(s) of your choosing] to meet with School representatives for an interactive process meeting. The need for this meeting process has been triggered by [state reason: e.g., your request for an accommodation, your doctor’s restriction, the results of the fitness for duty exam]. The meeting will occur at [date, time and place of interactive process meeting]. The purpose of the interactive process is to determine whether: 1) you have a disability that makes it more difficult for you to perform your essential work functions; and 2) identify all potential accommodations that might enable you to perform your essential work functions without causing undue hardship to the School and without the risk of substantial harm to your health and safety or the health and safety of others. During the meeting, we will identify and discuss: your work restrictions; potential accommodations that may enable you to successfully complete your job duties; your preferences; and potential options and alternatives that would provide effective accommodations.
