 Between October 1 and 15, file Form R-4, the Private School Affidavit, with the Superintendent of Public Instruction through the California Department of Education website ( http://www.cde.ca.gov/sp/ps/rq/affidavit.asp ). NOTE: The affidavit only needs to be filed once the school has at least one student.
