C HECKLIST W HEN A N E MPLOYEE R EQUESTS F AMILY A ND /O R M EDICAL C ARE L EAVE  Is the employee eligible for FMLA/CFRA leave?  Employed for at least 12 months?  Worked at least 1,250 hours during the preceding 12 month period?  Employed at a work site with at least 50 employees within 75 miles?  Does the employee have FMLA/CFRA leave time available?  Is there a qualifying reason for the leave?  Employee or family member’s serious health condition?  Bonding leave with newborn or recently placed adopted/foster child?  Qualifying exigency?  Care for a military member-family member with a serious injury/illness?  Did the employee give 30 days’ advance notice of need for leave, or as soon as practicable?  Has the school provided the required notices (see Section 4,G)?  General notice  Eligibility notice  Rights and responsibilities notice  Requirement to provide medical certification  Whether the school will require substitution of paid leave  Any requirement for employee to pay health care premiums  Various other rights and responsibilities (see Section 4, G)  Designation notice  Amount of leave counted toward FMLA/CFRA entitlement  Any requirement that employee provide a return to work medical certification  Provide list of essential job functions if return to work certification is required
